How to order a form 1 kit from Quietbore
We understand with our newest updates that ordering from us may need some extra guided steps. This post will walk you through exactly how to upload the required paperwork to complete your order.
Creating a Form 1
If you have not created or been approved for an ATF Form 1 yet we highly recommend having this completed and approved before ordering our kits. (you will be required to have a submitted form 1 prior to us shipping your order per ATF regulations)
The best/easiest way to create a form 1 is with the Eforms system which can be found at eforms.atf.gov, If you need help/guidance completing this form please click HERE FOR OUR ATF EFORM 1 WALKTHROUGH
Ordering From Quietbore
The two things we will need prior to shipping your complete kit or what the ATF dubs as main components (tubes and internals) are an approved or submitted Form 1 and copy of your drivers license/ proof of identification to verify you are the person / responsible person that matches the Eform. It is highly recommended to have the following on your computer and ready to upload before trying to place your order:
- Submitted or Approved form 1 Document in .pdf form (this will be sent to you via the eform system) See sample below
- Photo of drivers license or Government Issued ID in .jpg, .png or pdf form (can be a photo that is uploaded from your phone)
once you have these two items ready to go in approved file types you are ready to order.
You may submit an order with a “submitted” form 1
Checking Out
If you have a form 1 kit or parts listed below
- Complete Kits (all kits will be marked with the Form 1 information)
- Tubes (all tubes will be marked with the Form 1 information)
- Unfinished Internal Components
A separate form 1 verification will appear at checkout. You will then need to simply upload the two required files in either a .pdf, .jpg or .png to complete checkout.
Once both files are uploaded the section should look like the above. PLEASE NOTE: there is a 8mb max per file size to limit the load on the website.

Will you drill.out the silencer now for the consumer if the form 1 is approved?
unfortunately not since you cannot be physically present at our location for direct supervision.
I am in the process of getting everything ready to submit an E-Form 1 and purchase a Quiet Bore Kit, but I am confused as to what to put for Manufacturer/Model, is it supposed to be QuiteBore and 5.56/30 kit or is Manufacturer and Model whatever I want it to be?
At the manufacturer page you need to click on “by manufacturer code†then type FMI. Once you go to the next page you can select anything from that list that makes sense. Usually I recommend something simple like .30 cal or 5.56
How do I add the engraving service to my purchase?
Is the cover letter that gets sent after submission of the form 1 enough to place the order?
no, you need to use the submitted form (attached in the same email) so that way we can engrave your kit to match your form.
Would be nice if this page included an example of a Form 1 for a silencer and not one for a firearm to avoid mistakes, considering the process is very different for the 2 (e.g. length selection, manufacturer/model, etc.)
Just wanting to clarify, I can submit my completed form 1 document to you to place my order, BUT I will need to also send you the approved doc w/ stamp before you will ship it? When do you charge my card, at time of order or time of shipping?
you can use your submitted or approved form 1 doc to place and have us ship your order.
You are not REQUIRED to submit your approved form 1 for us to ship your order. Your card will be charged instantly like any other e-commerce site.
Shipping times are about 1-2 weeks currently depending on what you’re planning on ordering.

If I place an order with my submitted paperwork, I know that I can’t finish manufacture until I am approved. That said, once approved, do I need to follow up with you with approved paperwork?
No problem, you are not required to send it if we have already received your submitted form.
So if i am understanding this correctly, you CAN NOT ship an order without an approved or submitted form 1?
Correct. You need to supply at minimum a submitted form 1 and your DL. That way we can confirm and engrave your tube to match your order. We are an FFL/SOT and understand the “sensitivity” of personal info and we take care to make sure that that information is kept private and treated with the same care as Form 4 info and Firearm Transfer Paperwork.
How do/can you get replacement parts? I put your .45 cal can on my 223 n it blew it off after the 5-6 shot (semi-auto)
That depends on how messed up your tube and internals are…. you can either take It to an SOT and we can send completed parts to them, and they can replace / fix it or send it to us and we can take a look at it and get it fixed (we are also an SOT)
Will the 45 cal form 1 kit work with 450 bushmaster and 350 legend? And possibly 300 win mag
unfortunately not, it will not handle the additional pressures from those rounds.
I am ordering 2 kits. Your website only allows 1 form 1 upload. How do I handle multiple orders?
